Put your company name, logo, license number, and phone at the top, then add the customer and the address where the work was done. Give the invoice a unique number and set the issue and due dates.

How do I bill a call-out fee and labor on the same plumbing invoice?
Use separate lines. Put the service call or call-out fee on its own line, then add your labor as one or more lines with the hours and your hourly rate. List any parts you installed as their own lines with quantity and unit price. The template totals everything into a subtotal, adds tax, and shows one grand total, so the customer sees the full cost and the breakdown behind it.
How should I charge for an emergency or after-hours plumbing call?
Bill it the same way, just with your emergency rate. Note on the call-out line that it was an after-hours or weekend visit and use the higher rate you quoted, then add labor and parts as usual. Stating the emergency rate before you head out, and repeating it in the notes area, keeps the invoice from being a surprise.
